No fall protection for workers on the roof. There should be a fall protection system in place for workers on the roof, such as a fall restraint system to prevent workers from falling to a lower level.
Ladders should extend 3 ft above roof access point and secured (tied off)to prevent movement. Extension ladders are not designed to be used separately.
Workers should have appropriate PPE eye protection, work shoes, gloves, etc. Work area should be kept free of clutter
