Ladder not tied off to prevent sideways movement. Ladder less than 3 ft above roof access point. No fall protection, there should be a fall protection system in place, such as a fall restraint system to prevent workers from falling to a lower level. Worker does not have PPE, workers should be supplied proper PPE such as eye protection, gloves, harnesses, etc.
