Scaffold cross bracing missing. Scaffold set up on top of unsecured excavation. Scaffold legs/base should always be set up in a safe/secure area. No falling object protection for workers below scaffold,(toe boards, screens or debris nets). Poor housekeeping, numerous trip and/or fire hazards.
