This spreadsheet was created by Mike Harris, Ph.D. CIH of Hamlin & Harris, Inc. Dr. Harris is a welding expert and is sharing this industrial hygiene sampling form for welding to encourage more thorough and accurate collection of key data, such as the type of welding performed, the base metal, consumable type and size, as well as the position of the weld and the welder. This form has pull-down menus that direct industrial hygienists to collect more information than they might normally consider, particularly if they have limited experience with welding and cutting.
